1. What is your referral policy?
If you wish to obtain a referral to a specialist, this must be discussed with your doctor. We DO NOT provide referrals by phone. Once you have discussed the reasons with your doctor, the referral and appointment will be made for you. If you have not heard back from our office within 2 weeks, please call us to inquire about your appointment unless it is a diagnostic appointment (i.e. CT Scan or MRI). These types of appointments take a significant amount of time to book. Please be patient.
2. How do I find out about results from tests I`ve had done at Uptown Health Centre?
To obtain results of any tests (i.e. blood work, ultra sound, CT scan, x-rays, MRI) you must book an appointment with your doctor to review them. We DO NOT provide results over the phone. To ensure timely review of your test, please make an appointment with your physician at the end of your visit to the clinic.
3. What about follow-up appointments with my doctor at Uptown Health Centre?
If your doctor wishes you to book a follow up appointment, please book the appointment prior to leaving the clinic. This will ensure you receive your appointment in the amount of time your doctor wishes to see you again.
4. What about Prescription renewals?
When you see your doctor, please bring all your pills with you and review them with your doctor at each visit. You will be given enough medication to last until the next planned follow-up visit. Please ensure you have booked your follow-up appointment BEFORE you run out of your pills. On a one-time basis, if you have not arranged a follow-up appointment and do not have enough medication, your doctor may arrange for a refill of your prescription with your pharmacy for a fee of $20.00 which must be pre paid by credit card. Save yourself money and frustration by ensuring you have arranged an appropriate follow-up appointment before running out of medication.
Your may be use our online RX renewal request from. Please note that all renewal requests will be done at your doctor`s discretion. If a RX Renewal is not authorized by your doctor then your credit card will not be charged.
5. What happens if I miss an appointment?
As determined by the Ontario Medical Association, you will be billed $40 for an appointment ($100 for a physical examination) that is missed or cancelled with less than 24 hrs. notice.
6. Are all services covered by OHIP?
No, some services are not covered by OHIP. These include forms, notes, examinations that are being requested by third parties, travel medicine consultation and vaccinations, cosmetic treatments, chiropractic visits etc. Ask your doctor or our front desk staff for a complete listing if interested.
7. How do I become a patient at Uptown Health Centre ?
If you are interested in becoming a patient at the clinic please feel free to send us your name, address, and phone number at our new patients mailbox.
8. What is the Walk in and After Hours Clinic?
We hope to accommodate our patients with urgent needs through our Walk in and after hours walk in clinic. Accordingly, Monday to Friday from 9am to 8pm and Saturday and Sunday from 9am to 4.30pm one of our doctors is on duty for all patients. Although this may not be your regular doctor at the clinic your medical records can be accessed by the Doctor on duty, a follow up scheduled appointment with your regular Doctor may be recommended at the end of your visit.
9. What medical care is available outside office hours?
Outside of our regular opening hours you can call TeleHealth Ontario at 1-888 or click the link for medical advice.
10. What is Patient Access?
Some of our family doctors offer a limited online consultation service. This service is not covered by OHIP and does not replace your visit to the clinic. Click here to access the system.
11. How do I use the Online Prescription Renewal Service?
Although prescription renewals should be done in person during your visit, we provide a limited service which will enable you to receive a prescription for enough medication till your next visit. Limited renewals are are done at the discretion of your doctor at Uptown Health Centre. The fee for this service is $15 per limited renewal. You may use your VISA or MasterCard to process your request through our secure site.
12. Urgent Care Needs:
Please note that patients with medical emergencies should go straight to the Emergency Room of their nearest hospital or call 911 for assistance.
Telehealth Ontario is a free, confidential telephone service you can call at 1-866-797-0000 to get health advice or general health information from a Registered Nurse, and is available 24 hours a day, 7 days a week. |
